The Back Pain Golfers Ignore Until It Ends Their Season
A client came in last spring after four holes at Torrey Pines left him unable to tie his shoes the next morning. He was 47, played twice a week, walked five miles a round, and considered himself fit. His mistake wasn’t fitness — it was that his gym program had nothing to do with his golf swing. He was benching and curling while his thoracic spine barely rotated and his hips were locked up from a desk job in Carmel Valley.
That combination — a stiff upper back, tight hips, and a lower back doing work it was never built for — is the single most common pattern we see in golfers who train for strength training for golfers the wrong way. The swing asks the spine to rotate roughly 90 degrees at speeds exceeding 400 degrees per second while absorbing compressive loads up to eight times body weight at impact. If the hips and thoracic spine won’t rotate, the lumbar spine takes the load instead, over and over, for years.
This article lays out the exact rotational power program we run for golfers at our Del Mar studio: how we assess before we program, how the 12 weeks are structured, and why the anti-rotation core work matters as much as the power work. Why Distance Comes From the Hips, Not a Faster Swing
Most golfers chasing distance focus on swing speed drills or a stronger grip. Clubhead speed research points somewhere else entirely: the separation between hip rotation and shoulder rotation during the downswing, commonly called the X-Factor. The greater and faster that separation closes, the more elastic energy transfers through the torso into the club.
Think of it like a rubber band. The hips fire first and begin rotating toward the target while the shoulders and arms lag behind for a split second. That stretch across the obliques and thoracolumbar fascia stores energy, then releases it explosively as the upper body catches up. Golfers with restricted hip internal rotation or a stiff thoracic spine can’t create that stretch, so they try to generate speed with the arms and shoulders instead — which is slower, less powerful, and puts far more shear force on the lumbar spine.
This is why a program built around bicep curls and bench press does almost nothing for a golfer’s distance. The muscles that matter most are the glutes and hips (for rotational force production), the obliques and deep core (for transferring that force through the spine safely), and the thoracic spine (for the range of motion that makes separation possible in the first place). We build all three before we ever hand a client a medicine ball.
The Assessment: What We Measure Before Writing a Single Set
We don’t program rotational power for a golfer without first running three specific screens, because loading a restricted joint just teaches the body to compensate somewhere else — usually the lower back.
First is thoracic rotation, tested seated with hips stabilized, comparing left and right sides. A meaningful side-to-side asymmetry (more than about 10 degrees) tells us which direction needs mobility work before we load rotation in that plane. Second is hip internal rotation, tested lying face-down with the knee bent — the lead hip in particular needs 35-45 degrees to clear properly through impact without the lower back compensating. Third is anti-rotation core endurance, using a modified side plank or Pallof hold to failure, which tells us how long the deep core can resist rotational force before it breaks down under fatigue, which is exactly what happens over 18 holes.
These three numbers become the baseline. We retest at week 6 and week 12, the same way we’d retest any client working toward a measurable goal — the same logic behind our 90-day strength benchmark testing we run for every program. A golfer who can’t crack 30 degrees of hip internal rotation on the lead side isn’t ready for loaded rotational throws yet, no matter how motivated they are to add yards before their next member-guest.
The X-Factor Explained — and Why It’s Also What Hurts Your Back
Here’s the tension every golf fitness program has to resolve: the same hip-shoulder separation that generates distance is also the movement pattern most associated with golf-related low back pain when it’s trained without control. A review published in the Asian Journal of Sports Medicine found lumbar rotation and lateral bending under repetitive load to be primary contributors to the condition, which affects a significant share of amateur golfers over a season of regular play (Lindsay & Vandervoort).
The resolution isn’t to avoid rotation — it’s to source it correctly. Rotation that happens through the hips and thoracic spine, with the lumbar spine staying relatively stable, is safe and powerful. Rotation that happens because the lumbar spine is compensating for stiff hips and a stiff upper back is what accumulates into injury. Practically, this means every rotational power exercise in the program is paired with a joint-specific mobility drill for whichever screen came back restricted, and loading always progresses from bodyweight to light med ball to heavier resisted work — never straight to load.
The 12-Week Program Structure, Phase by Phase
The program runs in three four-week blocks, each with a distinct job. This isn’t arbitrary — it mirrors how motor learning and tissue adaptation actually happen, the same micro-progression logic we apply across every strength program we build, outlined in our piece on designing incremental strength progressions.
Weeks 1-4 — Foundation. The goal is restoring thoracic and hip rotation and building anti-rotation core endurance. Two sessions per week, 40-45 minutes. No golf-specific power work yet — this phase is about closing the mobility gaps the assessment flagged.
Weeks 5-8 — Rotational Power. Two to three sessions per week. Med ball rotational throws, cable chops and lifts, and loaded hip hinges enter the program. Volume and intensity increase progressively — we’re not maxing out week 5, we’re building toward a peak in week 8.
Weeks 9-12 — Speed and Transfer. Sessions shift toward lower volume, higher velocity work. This is where we add speed-focused rotational throws and start pairing gym sessions with range work so the new strength pattern shows up in the actual swing, not just in the gym.
Each block ends with a light deload — roughly 40% volume reduction — before ramping into the next phase, which keeps fatigue from masking the strength gains underneath it.
Sample Training Week: Exercises, Sets, Reps, Tempo
Here’s what a typical Weeks 5-8 session actually looks like, run twice weekly with 48-72 hours between sessions:
- Half-kneeling cable chop: 3 sets of 8 reps per side, 2-second controlled eccentric, explosive concentric
- Standing rotational med ball throw against wall: 4 sets of 5 throws per side, full recovery between sets (60-90 seconds) since this is a power movement, not a conditioning one
- Trap bar deadlift: 4 sets of 5 reps at 70-75% of tested max, 3-second eccentric tempo
- Single-leg Romanian deadlift: 3 sets of 8 reps per side, focusing on hip control through the transition
- Pallof press with rotational step: 3 sets of 10 reps per side
- Suitcase carry: 3 sets of 30 yards per side, heavy enough that lateral trunk flexion is a real challenge to resist
Notice what’s absent: no bicep curls, no crunches, no leg extensions. Every movement either builds rotational force production or teaches the core to resist unwanted rotation and lateral flexion — the two qualities that actually change a golfer’s swing and protect the lower back at the same time.
Anti-Rotation Core Training: The Piece Most Golf Programs Skip
Golfers naturally gravitate toward the exciting part of the program — the med ball throws, the speed work. The anti-rotation training is less exciting and gets skipped first when a client is short on time. That’s a mistake, because anti-rotation strength is what determines whether the power work translates to distance or to a physical therapy referral.
Anti-rotation exercises train the deep core — obliques, transverse abdominis, multifidus — to resist unwanted movement rather than create movement. A Pallof press, for example, asks the core to prevent rotation while a cable pulls the torso sideways. A suitcase carry asks the same muscles to prevent lateral flexion while walking under an offset load. Dead bugs and bird dogs build the same quality lying down or on all fours, useful for golfers just starting to rebuild capacity after a flare-up.
We program anti-rotation work in every phase of the 12 weeks, not just the foundation phase, because it’s the stabilizing counterweight to the power training that comes later. A golfer who can throw a med ball explosively but can’t hold a 45-second Pallof press is generating power the spine can’t control — that’s exactly the profile that shows up with back pain by month three of an unbalanced program.
Mistakes We See Constantly From North County Golfers
The most common mistake is skipping the assessment and jumping straight to rotational throws because they feel golf-specific and satisfying. Without knowing hip and thoracic rotation numbers first, a coach is guessing, and the lumbar spine usually pays for the guess.
The second mistake is training rotation with load in the same plane and range every session — usually a twisting cable exercise done the same way, three sets, every single week for months. Tissue adapts to novel stimulus; without progression in load, tempo, or range, golfers plateau by week 6 and stop seeing speed gains.
The third, and most common among the busy professionals we work with along the Solana Beach and Encinitas corridor, is inconsistency. A golfer trains hard for two weeks before a trip, then does nothing for a month, then wonders why the gains didn’t stick. Rotational power and motor pattern changes require consistent exposure — two sessions a week for 12 straight weeks beats six sessions crammed into one week. What to Expect at Week 4, 8, and 12
By week 4, most clients report noticeably less stiffness getting in and out of the car after a round, and their thoracic rotation screen usually improves by 8-12 degrees. This is mobility and motor control catching up, not yet a strength or speed change — don’t expect more distance yet.
By week 8, clubhead speed changes typically become measurable, often in the range of 3-6 mph on a launch monitor, driven by the increased hip-shoulder separation and stronger force production through the hips.
